Medical Examination Scheduling Process for Immigrant visa applicants for the U.S. Embassy in Tashkent

Tashkent International Clinic (TIC) conducts a medical examination for applicants who have applied for DV (Green Card) Lottery, Reunification (K1/K2 fiancé and other family members’ reunification) and other Immigration visas

As soon as you receive an Invitation Letter for an interview at the U.S. Embassy in Tashkent, you need to pay for the medial examination at www.intms.org

Please see below step-by-step instructions on how to pay for medical services, how to make an appointment for the medical examination, and which documents you are requested to bring with you.

International Medical Services website (www.intms.org ) is portal for payment for medical examinations for Immigration visa applicants traveling abroad. The United States, Canada, and the United Kingdom are listed as countries to choose from on the website. You may choose a language (Russian or English) for your convenience and start filling out the questionnaire.

Note: Family members whose names are not indicated in the Invitation letter cannot be admitted to the medical examination. Please contact the U.S. Embassy in Uzbekistan to find out how to add family members to the invitation letter.

Once you received the payment confirmation to your email, you need to send the required documents listed below in scanned PDF files and photos for each family member in .jpeg format to the email medicalsTIC@tashclinic.org .

You will receive an appointment date for your medical exam via email, once an operator checks and registers your documents in the system. All other questions and inquiries regarding medical exams are resolved by emailing medicalsTIC@tashclinic.org only.

All scanned documents must be in good quality in PDF format only.

List of required documents for registration:

  • Traveling passport original and one copy (in good quality) for every family member
  • Photo, sizes 3x4 cm in JPEG format only (color, white background and photo taken minimum of 6 months prior)
  • Invitation Letter from the U.S. Embassy in Tashkent
  • DS-260 form (Online Immigrant Visa and Alien Registration Application) for immigration, reunification visas and DV Lottery visas
  • DS-160 form (Online Immigrant Visa and Alien Registration Applicant) K1/K2 fiancé(e) visas
  • 063 medical certificate from local polyclinic or vaccination records certificates from any vaccination centers (if applicable). This document does not need to be notarized. Please note that the vaccination records worksheet from Tashkent International Clinic will be sent to your valid email address within a month after the medical examination.
  • Covid-19 vaccination certificate
  • Your current resident address, the U.S. resident address and email address listed on a WORD document and sent as a scan.

The U.S. Centers for Disease Control and Prevention (CDC) requested that all applicants aged 2-14 inclusive must take the IGRA test (15 ages and older are not required to take the test). The IGRA test is a diagnostic tool for active or latent TB infection. The test is performed by blood taken from a vein.

After receiving an email with the date and time of the medical examination, you will need to make an appointment for children from 2-14 years old for the IGRA Test immediately. The test procedure may take from 2 to 5 days, and obtaining and providing the test results to the Tashkent International Clinic is the applicant's responsibility. The result of the test should be sent in advance by e -mail: medicalsTIC@tashclinic.org or provided during the medical examination.

Children from ages 2-14 inclusive (15 years old and older are not required to take the IGRA test) who do not have the IGRA test will not be permitted for a medical examination.

List of documents that you need to bring with you on the day of the medical examination:

  • Traveling passport original and copy in good quality for each family member who is scheduled for medical appointment day;
  • Invitation Letter to the U.S. Embassy in Tashkent;
  • 063 medical forms original and copies from local polyclinic or vaccination certificates and copies from Vaccination Centers for each family member;
  • Covid-19 Vaccination Certificate for each family member older than 12 years old, inclusive
  • DS-260 form (Online Application for Immigrant Visa and Alien Registration) form for DV Lottery and Immigration Visa applicants;
  • DS-160 form (Online Application for Immigrant Visa and Alien Registration) K1/K2 (Fiancée/Fiancé) visa applicants;
  • IGRA test results for children from 2-14 years old (inclusive)
  • Eye glasses or contact lenses (if applicable)
  • Listed on paper – 3 addresses: Local resident address, U.S. resident address and email (current) address

Please arrive at the appointed time. Do not come early and do not come late. Fasting is not required.

Terms and Conditions during medical examination day at Tashkent International Clinic

Applicants should arrive according to the scheduled appointment time with their passport and any other documents related to their medical exam. The Security Guard of the clinic will call the applicant according to the schedule for a security check.

Families with small children, pregnant women, and the elderly will be passed through the Security check first as a PRIORITY

To the attention of Pregnant applicants:

All applicants who undergo medical exam for Immigration or Non Immigration (K1/K2) visas must undergo all steps of medical check in order to complete the exam. According to Technical instructions by U.S. Immigration Center, all Applicants who are pregnant may postpone the required immigration chest x-ray (and immigration medical examination) until after pregnancy but are required to have a chest x-ray to immigrate.  Pregnant applicants should discuss this matter with the Department of State (DOS) consular officer to determine if the delay will affect their immigration status. https://www.cdc.gov/immigrantrefugeehealth/panel-physicians/tuberculosis.html#chest

In case of pregnancy, TIC recommends applicants to discuss all risks and probabilities with their treating doctor (OBGYN or family physician) and have a decision before coming to medical exam. If applicants refuse to undergo chest x-ray, medical check is postponed until Chest X-ray is done. As an option, applicants can request medical check after giving birth. All other related inquires as how to reschedule for medical exam and how can it be done, must be directed to the Consular Section of the U.S. Embassy in Tashkent at TashkentIV@state.gov

INFORMED CONSENT FOR X-RAY EXAMINATIONS

The medical examination may last from 2 to 4 hours.

If you have children, please bring baby food, diapers and their small toys and books.

Video Recording: Please note that all activity in public areas is being monitored and recorded by clinic security cameras, including microphones recording what is being said.

The Security Guard will explain the rules, procedures and requirements to enter the territory of the clinic. Applicants will be asked to switch off their mobile phones and hand over their electronic devices to the Clinic Security services, including belongings as car keys, liquid substances, sprays and others.
