Medical Examination Processing

Your visa application requires a medical exam. The Tashkent International Clinic will provide the results of your medical exam to Canadian Immigration and Citizenship (CIC).

Note: Before continuing on to the next screens, you will need the following:

Step 1: Personal Information. After reading through the steps and agreeing to the terms and conditions, continue to the next two screens to fill in all the needed personal information and upload your photo.

Step 2: Payment. After you have uploaded your photo and filled in all the necessary personal information you will be directed to the payment portal for IntMS (www.intms.org)  which automatically calculates the amount due for you individually or the total due for you and your family.

Please Note:  TIC does not collect charges, the payments for medical exams are directed to International Medical Services.

The non-refundable cost of a medical exam and all required lab analysis is:

  • Applicants 0 – through 4 years old – $105.
  • Applicants 5 years old through 10 years old – $117.
  • Applicants 11 years old through 14 years old – $157.
  • Applicants 15 years old and older – $242.

You may pay the fee using a MasterCard or Visa.

  • Make sure that the card holder’s name, card number and expiration date are correct.
  • Make sure there are enough funds on the payment card.
  • If your card is denied, check with the issuing bank of the card to find out why it is being denied.

After making the payment, a receipt and the information you have filled out will be sent to the e-mail address you specified in the form and to the Tashkent International Clinic.

Step 3: Scheduling your appointment for a Medical Exam

After you have received confirmation of payment, call the Tashkent International Clinic to schedule your appointment. The phone number is: +998-78 120-1120 Calls are received between 8:30 am to 5:00 pm Monday through Friday.

Step 4: The day of your appointment

On the day of your appointment, arrive on time. Do not come early and do not be late.

Bring the following documents with you:

  • Valid passport for each person in the family
  • 2 copies of the Passport(s) picture page

  • Medical Report (IMM 1017) with your IME # or if you do not have an IME # then your IMM 1017B UPFRONT document.
  • All current and past medical and Vaccination records

Terms and Conditions

You are responsible for the validity and accuracy of all the information you provide on this form.

Applicants should arrive according to the scheduled appointment time with their passport and any other documents related to their medical exam.

Applicants must abide by the rules, procedures and requirements to enter the clinic territory. The applicant will be asked to switch off their mobile phone and pass all electronic devices, including car keys, any liquid items, sprays, etc. to the security guard.

Video Recording: Please note that all activity in public areas is being monitored and recorded by clinic security cameras, including microphones recording what is being said.

Payments will be processed according to the fee schedule outlined in step 2. Payments are non-refundable.
