Medical Examination Process for those applying for student, work, visitor visa to the Kingdom of Belgium.

Tashkent International Clinic provides medical examination to the applicants applying for visa to Belgium. See the instructions below how to make an appointment, how to make the payment and list of documents requested for the medical examination.

Before making an appointment at the Clinic for a medical examination, payment must be made on the following website:

Note: For payment, you will need the following list of documents:

  • Recent Electronic Photo 3 x 4 cm to upload (In color, white background, taken within the last six months)
  • Passport (Red or traveling) to complete personal information
  • E-mail address
  • Master Card or Visa credit card

Depending on the country where you are applying for a visa, select the appropriate flag of the country (Kingdom of Belgium) and start filling out the questionnaire.

Below is the instruction how to fill up the form:

Step 1. Read the “Terms of Agreement” carefully. If you agree to the terms for the provision of personal information, please check the box (√) and proceed to the next step.

Step 2. Enter number of people who will undergo medical examination.

Step 3. Enter all family members’ traveling passport data and upload the photo.

After filling out the form correctly, you will be redirected to the payment portal for international medical services, which will automatically calculate the cost for both the individual and the total cost of the medical examination for you and your family.

Please note: Tashkent International Clinic does not charge for medical examination, the payment is sent to the service of International Medical Services.

The cost of a medical examination is:

  • For applicants 0-10 years old – 50 USD.
  • For applicants 11 years of age and older – 90 USD.

At this stage, international bank card – Visa or MasterCard is required:

  • Make sure the name and last name of cardholder, card number and expiration date are correct.
  • Make sure the card has sufficient funds on it.
  • After payment is successful, the receipt and the information you filled in will be automatically sent to the email address of the Tashkent International Clinic, as well as to the email address that you indicated when filling out the form.
  • If the payment request is rejected, contact the bank provided the card to clarify the reason for the payment denial.
  • Payment is non-refundable.

Step 5. Appointment for a medical examination.

After you receive payment confirmation, please call the Tashkent International Clinic to make an appointment at the number: + 99871 291-07-26/ +99878 120-11-20. Calls are accepted from Monday to Friday from 10:00 to 12:00 and from 14:30 to 17:00.

Step 6.  On the day of the medical examination IT IS NECESSARY TO COME STRICTLY AT THE APPOINTED TIME, do not come early, but do not be late either. There is no need to come on an empty stomach.

List of required documents for medical examination:

  • Valid traveling (red) passport and, for each family member if you are traveling with your family
  • 1 copy of traveling (red) passport (main page with photo) for each family member
  • A job offer/study invitation letter from Belgium University

You are responsible for the validity and accuracy of all the information you provide on this form.

Applicant should arrive according to the scheduled appointment time with their passport and any other documents related to their medical exam.

Applicants must abide by the rules, procedures and requirements to enter the clinic territory. The applicant will be asked to switch off their mobile phone and pass all electronic devices, including car keys, any liquid items, sprays, etc. to the security guard.
